Members to submit Investment Option Change Form (“the Form”) ¢w HOW? |

Form Download
The Form is obtainable from the above ¡§Download Area¡¨ throughout the year. |
| b. |
Submission Deadline
The Form submission deadline will normally fall between the 19th and the 22nd of the month immediately preceding the option change effective date. Members should observe the submission deadlines for respective months by referring to the following timetable (which is also listed in the Note of the Form):-

Completion of the Form
For a valid option change instruction, members should indicate clearly on the Form the ¡§Fund Switching Effective Date¡¨ and the fund allocation %. The University and Trustees shall not be liable for any delay or loss whatsoever or howsoever arising from an incomplete instruction. |

Submission of the Form
The completed Form should be returned for receipt by the Payroll & Superannuation Unit of Bursary by mail or fax (2603-7904) or email (s95enquiry@uab.cuhk.edu.hk) on or before the closing date listed in the above note 2b. The University and Trustees will not accept any claims for loss in relation to the non-receipt of this Form. |

Acknowledgement of the Form
Should members require email acknowledgement receipt of the Form, please provide their email address where indicated on the Form. If provided, acknowledgement will be sent to your email address within two working days.
In addition, members who have exercised option change will further receive confirmation letter of new investment allocation about one week after the respective closing dates.
