  IMPORTANT: Please read thoroughly the following instructions before starting to work on the Online Course Selection Form.  
     
  Step 1 - Login to your Online Application Form  
  Step 2 - Input course information into your Course Selection Form  
  Step 3 - Save/Submit your Online Course Selection Form  
  Step 4 - Review and Amend your Submitted Form  
     
 
 
     
  Step 1 - Login to your Online Application Form  
     
  1.1 Please login to your online application form with your email address and password. If you studied at CUHK in the last term, please login to your online application form with your CUHK email address (i.e. "s" + 10 digits of Student ID+ "@mailserv.cuhk.edu.hk") and input your date of birth as the password (e.g. if your birth date is 1 July 1980, please input "010780" as your password).  
     
       
  1.2 Choose your application.  
     
     
     
  Step 2 - Input course information into your Course Selection Form  
     
  2.1 Click the link to your course selection form provided in the “My Task” section.  
     
       
  2.2 Input CUHK course code for the course which you would like to take, e.g. HIST3212-.  
     
       
  2.3 Input 1st Term or 2nd Term.  
     
       
  2.4 Click “Insert” to confirm.  
   

 
   
 
     
       
  2.5 Repeat point 2.1-2.4 to add up to a maximum of 18 units in the table.  
       
     
  Step 3 - Save/Submit your Online Course Selection Form  
       
  3.1 Click "Save/Submit" and then "OK" to secure your input and send CUHK your Online Course Selection Form.  
     
       
  3.2 Click "Close" and then "OK" to exit the form.  
     
     
     
  Step 4 - Review and Amend your Submitted Form  
       
  1. Use your email address and password to login to the system and view your course selection form.  
  2. Amend the data if necessary. Please note that no amendment will be allowed after the deadline mentioned below.
