Students who wish to apply for minor in Physical Education and Sports Science should complete this form, and the prescribed form obtainable from the Registration and Examinations Section or may be downloaded from RES hompage: http://www.cuhk.edu.hk/rgs. The completed forms together with your University’s academic report(s) should be returned to our General Office on or before July 10. Students will be notified the result by telephone on or before July 12.

The forms can be obtained starting from June 4. The approved application should be returned to the Registration and Examinations Section on or before July 18.
