Nomination and Recommendation for Nomination

Nomination(s) should be submitted by Department Chairmen/Unit Heads. The nomination form may be downloaded here.
All CUHK staff members and students are also welcome to recommend eligible staff membres to compete for this Award. The recommendations will be reviewed by related Department Chairmen/Unit Heads before making final nominations. The recommendation for nomination form may be downloaded here, or submitted through the Online Recommendation for Normination Form directly.
Please complete and send the Form to the Secretary, Selection Panel of the Exemplary Service Award 2012-13, c/o Personnel Office, 4/F, Mong Man Wai Building, on or before the deadline.
